UHS have now developed an online form which patients (or their carers) can complete to let us know if they have any accessible information needs when coming in to their hospital (such as interpreters, letters in braille/large print etc).

Once the form is completed, an alert will be added to their electronic Trust records which will flag these needs to our staff. They can also email accessible@uhs.nhs.uk for advice or information about this.
